End stage renal disease (ESRD) prevalence has steadily increased in the United States with over 341,000 individuals receiving in-center hemodialysis (HD) treatment which is the most common therapy (Russell and others, 2011). Hyperphosphatemia, one of the clinical consequences of ESRD, is associated with a numerous serious complications, such as vascular calcification and left ventricular hypertrophy leading to cardiovascular diseases, as well as 20-40% increase in mortality risk (Huml, Sulliva, Leon, and Sehgal, 2012). Managing hyperphosphatemia remains to be a challenge among renal clinicians. The Dialysis Outcomes and Practice Patterns Study (DOPPS) revealed that non-adherence significantly increases the risk of hospitalization and mortality among patients with ESRD according to Russell and others who suggested that motivational interviewing has been effective in changing behavior related to chronic disease and substance addictions. Furthermore, a nurse-led study conducted by Shi and others (2013) revealed statistical evidence identifying intensive education as an effective intervention in managing recurring hyperphosphatemia. Applying the concepts of motivational interviewing and patient-centered approach to care, 60 patients with re-occurring hyperphosphatemia will undergo a six-month patient education program incorporating the Health Belief Model theory to facilitate behavioral changes often associated with patient non-adherence (Syx, 2008). This proposed nurse-led patient education project seeks to identify patient information need that will help facilitate adherence to phosphate binder administration using the goal setting method. Staff education and training will take place prior to program initiation. Surveys will be conducted throughout the study and serum phosphate levels will be evaluated on a bi-weekly basis. Findings of this proposed project will be presented to the staff of the respective dialysis center then to its affiliated centers. Upon the organization's discretion, abstract proposal will be submitted for verbal or poster presentation through local and national renal conference or convention. Hyperphosphatemia is a serious condition that compromise patient's health and quality of life (Lindley, 2009). With continuous patient-centered education by dedicated dialysis staff nurses in collaboration with physicians, dietitians, and social workers, management of serum phosphate levels will be achieved thus impacting patient outcome as well as clinic performance.
