One of the most important aspects of home health nursing is patient education. Unfortunately, many patients who are discharged from the hospital lack a clear understanding of discharge instructions, including what medications to take and when to follow up with their primary care physician. (Reducing Hospital Readmissions with Enhanced Patient Education, 2010). The role of the home health nurse is to help reduce the incidence of hospital readmissions and one way to do this is through patient education. The home health nurse can contribute to continuity of care by educating patients on ways to manage their illness or condition at home by instructing on medications, diet and exercise modifications and proper use of equipment such as glucometers and blood pressure cuffs. The problem is that in order to be effective teachers, nurses need to be comfortable in their teaching role and much of the education for nurses is geared towards performing technical tasks and there is less focus on how to communicate with patients. Until there is a change in the nursing school curriculum, home health agencies and other facilities can help staff nurses to be confident in their teaching skills by conducting role playing and simulation exercises that mimic actual teaching sessions with patients, particularly those with congestive heart failure (CHF). These patients require ongoing education in order to manage their condition and a nurse who is able to use different teaching approaches and educational tools will have a better chance to achieve patient satisfaction and positive patient outcomes. There is an opportunity for agencies to provide the needed education to nurses by offering classes and mentoring programs. Once programs are in place, they can be evaluated by calculating any hospital readmissions and conducting patient and nurse surveys.
